Patient: 51 year-old male with Type II diabetes; hypertension; cholesterol; problems with sleep; well controlled in all aspects via regular visits to physician; Prescriptions: Lipitor; Altace; Promethazine –for sleep

Chief Complaint: “Pain when I bite”; “I really don’t want to lose any more of my teeth”

Dental History: The patient presented with a sudden development of biting sensitivity on his lower right first premolar (Tooth #28). The treatment on the said tooth had been completed approximately 2 years prior with nonsurgical root canal treatment, a post/ core and crown.
