What are the long term affects of a delayed core and post placed without a rubber dam?

Patient: 44 year old female in good health (1988)

49 years old when referred a second time (1993)
66 year old female with glaucoma at last recall (2010)

Chief Complaint:

Asymptomatic (1988)
Asymptomatic (1993)

Significant Dental History:

1988: The patient was initially referred for the treatment of tooth #18 after her referring dentist prepared her tooth for a crown which resulted in a caries exposure during the caries control phase. The patient presented with numerous crowns with extensive build-ups throughout her mouth.

1993: The patient returned for examination of tooth #19. Some old restorations had been replaced and new crowns made since 1988.
