The advent of CBCT technology for routine dental imaging has led to the discovery of complex root forms like the fused distobuccal-palatal root in maxillary molars which are troublesome for endodontic treatment

Case: Patient presented on recall with a complex anatomic form

Patient: 65-year-old female with mild hypertension and an irregular heartbeat

Chief Complaint (1999): Severe cold sensitivity after a crown preparation (#14)

Dental History (1999): Deep caries, history of crown fracture and a deep base placed during crown preparation (#14). The referring dentist referred the patient for an endodontic evaluation after symptoms developed.
