Patient: 61 year old female in excellent health

Chief Complaint: Biting Sensitivity (upper left posterior jaw)

Dental History: Nonsurgical endodontic therapy performed by the author in 1986 (Teeth #’s 14 and 15), no recent dental restorations, finished Invisalign treatment in 2008
