When should a third molar be endodontically treated?

Patient: 94 year-old female (at time of recall, 2013) with controlled hypertension, a pacemaker, polycythema vera, and hypothyroidsim

Chief Complaint: biting sensitivity on the back upper left molar (tooth #16) (1996)

Dental History: In 1986 the patient had a gold crown placed on tooth #16. The maxillary left first molar had been lost (tooth #14). Root canal treatment had been performed on the maxillary left second molar (tooth #15) which served as a distal bridge abutment (#13-15). The patient also reported a history periodontal disease and constant management with cleanings every 3 months.
