Patient: 53-year-old woman in excellent health (2013)

Chief Complaint: Bump on gums

Dental History: Root canal treatment had been previously performed on the maxillary right lateral incisor (#7) in Bakersfield, California (1989) by a general dentist. The treatment had reportedly failed and was retreated with apical resection and reverse filling by an endodontist in Peru. A porcelain-fused-to-metal crown had been placed on the tooth in Peru (2001). The author retreated her #9 tooth in 2003 and noted that #7 also had a periapical radiolucency. It was examined in 2004 and 2005 as well. Her local dentist noted a periapical radiolucency and labial draining fistula associated with the tooth and referred her to the author’s practice for evaluation in 2012
