Retreatment of previously ledged and blocked root canal systems can be challenging with unpredictable results.

Case: Patient presented for endodontic retreatment with a suspected root fracture

Patient: 68 year old female with controlled hypertension and penicillin allergy

Chief Complaint: localized biting sensitivity mandibular left first molar

Dental History: The previous root canal treatment had been performed at least 2 decades prior to the consultation. A throbbing dull ache had started 3 days prior to the patient’s appointment, which was exacerbated by biting.
